ABSTRACT

SUMMARY: Craniofacial symmetry is an important factor in creating a harmonious facial appearance. Genetic and external factors may cause the formation of mandibular asymmetry. The aim of this study was to evaluate vertical mandibular asymmetries in adolescents who had unilateral mandibular first permanent molar (FPM) teeth extracted at an early age. The study group consisted of 60 subjects (30 females, 30 males with a mean age of 16.18±1.04 years) who had their mandibular permanent first molar tooth extracted before the age of 12, and the control group consisted of 60 healthy subjects (30 females, 30 males with a mean age of 16.23±0.92 years). Condylar asymmetry index (CAI), ramal asymmetry index (RAI), and condylar-ramal asymmetry index (CRAI) were calculated using panoramic radiographs of the subjects. Independent samples t-test was used to evaluate the differences between groups. CAI, RAI, and CRAI values were similar between male and female subjects in both control and study groups, and no statistically significant difference was found (p>0.05). No statistically significant difference was observed between the group who had their mandibular first permanent molar teeth extracted at an early age and the control group (p>0.05). CAI values were relatively higher in both groups, but there was no significant difference between the CAI, RAI, and CRAI values between the groups.


La simetría craneofacial es un factor importante para crear una apariencia facial armoniosa. Factores genéticos y externos pueden causar la formación de asimetría mandibular. El objetivo de este estudio fue evaluar las asimetrías mandibulares verticales en adolescentes a quienes se les extrajo el primer molar permanente (FPM) mandibular unilateral a una edad temprana. El grupo de estudio consistió en 60 sujetos (30 mujeres, 30 hombres con una edad media de 16,18±1,04 años) a quienes se les extrajo el primer molar mandibular permanente antes de los 12 años, y el grupo control consistió en 60 sujetos sanos (30 mujeres, 30 hombres con una edad media de 16,23±0,92 años). El índice de asimetría condilar (CAI), el índice de asimetría ramal (RAI) y el índice de asimetría condilar-ramal (CRAI) se calcularon utilizando radiografías panorámicas de los sujetos. Se utilizó la prueba t de muestras independientes para evaluar las diferencias entre los grupos. Los valores de CAI, RAI y CRAI fueron similares entre los hombres y las mujeres tanto en el grupo control como en el de estudio, y no se encontraron diferencias estadísticamente significativas (p>0.05). No se observaron diferencias estadísticamente significativas entre el grupo al que se le extrajo el primer molar permanente mandibular a una edad temprana y el grupo control (p>0,05). Los valores de CAI fueron relativamente más altos en ambos grupos, pero no hubo diferencias significativas entre los valores de CAI, RAI y CRAI entre los grupos.

ABSTRACT

Introduction: Mandibular fractures are the most common facial fractures affecting various anatomical sites of the mandible. Among the various mandibular fractures, management of condylar fractures remains a challenging task for surgeons. Case Report: We report the case of a 28 year old male patient who presented with pain in the chin and restricted mouth opening. Computed tomography revealed a sagittal fracture of the right condylar head with medial displacement of the fractured fragments. Management of diacapitular fractures includes open reduction and internal fixation of the right condyle using a single lag-screw. Results: The postoperative outcomes were favorable, where normal mandibular movements, desired dental occlusion and exact positioning of the condyle with rigid fixation were established thereby maintaining the shape of the condyle. Conclusion: Use of single lag screw fixation is highly recommended as it greatly supports the stabilized fracture fragments and also aid in prevention of fracture fragment rotation medially.


Introducción: Las fracturas mandibulares son las fracturas faciales más comunes que afectan a diversos sitios anatómicos de la mandíbula. Entre las diversas fracturas mandibulares, el manejo de las fracturas condilares sigue siendo una tarea desafiante para los cirujanos. Reporte del Caso: Presentamos el caso de un paciente masculino de 28 años que consultó por dolor en el mentón y restricción de la apertura de la boca. La tomografía computarizada reveló una fractura sagital de la cabeza condilar derecha con desplazamiento medial de los fragmentos fracturados. El tratamiento de las fracturas diacapitulares incluye la reducción abierta y la fijación interna del cóndilo derecho con un solo tirafondo. Resultados: Los resultados postoperatorios fueron favorables, donde se establecieron los movimientos mandibulares normales, la oclusión dentaria deseada y el posicionamiento exacto del cóndilo con fijación rígida manteniendo así la forma del cóndilo. Conclusión: Se recomienda encarecidamente el uso de una fijación con un solo tornillo de tracción, ya que soporta en gran medida los fragmentos de fractura estabilizados y también ayuda a prevenir la rotación medial de los fragmentos de fractura.

ABSTRACT

Objective: To investigate the imaging features of condylar cystic degeneration of temporomandibular joint (TMJ) by cone-beam CT (CBCT), spiral CT, MRI and radionuclide bone imaging. Methods: From January 2018 to December 2020, thirty-two patients with cyst-like lesions of condylein temporomandibular joint were examined by CBCT, spiral CT, MRI and radionuclide bone imaging at the Department of Oral and Maxillofacial Surgery in General Hospital of Chinese PLA. There were 12 males and 20 females involved, aged from 16 to 65 years with an average age of (33.9±12.5) years. The characteristics of CBCT, spiral CT, MRI and radionuclide bone imaging were analyzed. Condylar cyst like lesions were classified as type A and type B based on the presence or absence of surface bone defects. Condylar cyst like lesions were classified as type Ⅰ(yes) and type Ⅱ(no) according to the accompanying bone marrow edema-like lesions of the condyles. The incidence of condylar bone marrow edema, disc displacement and abnormal bone metabolism were analyzed. Results: A total of 64 joint images of 32 patients were included, including 34 sides with TMJ cyst-like lesion and 6 sides with multiple cyst-like lesions,the total cyst-like lesions were 42. The largest diameter of cyst-like lesion ranged from 1.0 to 12.4 mm, with an average length of (3.7± 1.8) mm. There were 24 cases of type A TMJ cyst like lesion and 10 cases of type B cyst-like lesion. The detection rate of CBCT was 95.2% (40/42) and that of spiral CT was 100% (42/42), there was no significant difference (Calibration Chi-square=0.51, P=0.474). The detection rate of nuclear magnetic resonance was 80.1% (34/42), and the detection rate of cyst-like lesions less than 2 mm was 3/11. In the cyst like lesion side, there were 9 sides with anterior disc displacement with reduction, 20 sides with anterior disc displacement without reduction. In the non-cyst like lesion side, 10 sides with anterior disc displacement with reduction and 6 sides with anterior disc displacement without reduction. There was a significant difference in the displacement of the disc between cyst-like and non-cystic lesion side (χ²=7.80, P=0.005). MRI showed that 6 cases of cystic side[17.6% (6/34)] had bone marrow edema-like lesions (all type A), 1 case of non-cyst like lesions side [3.3% (1/30)] had bone marrow edema-like lesion, there was no significant difference between cystic and non-cystic lesions (Calibration Chi-square=2.04, P=0.153). There was a significant difference between type A and B cystic lesions (Fisher exact probability method, P=0.024). Radionuclide bone imaging showed abnormal bone metabolism in 26 patients in the cyst-like lesion side and 5 patients in the non-cyst like lesion side (χ²=22.82, P<0.001). Conclusions Multi-slice Spiral CT could detect the cyst-like lesion of TMJ condyle in the early stage, which is different from the large joint. And the formation mechanism may vary from the different classifications.

Resumen Objetivo. Comparar posición condilar y espacio articular en articulación témporo-mandibular, en pacientes con y sin disfunción témporo-mandibular mediante tomografía Cone Beam. Métodos. Criterios diagnósticos de investigación para trastornos témporo-mandibulares Eje II, n=50 pacientes ambos sexos, edad comprendida 18 - 27 años. Muestra aleatoria n= 50 pacientes equivalente a 100 articulaciones, se obtuvo: n=25 pacientes sin disfunción témporo-mandibular, considerados asintomáticos 25 con y sin disfunción témporo-mandibulares, considerados sintomáticos. Mediante tomografía Cone Beam de articulación témporo-mandibular boca abierta - boca cerrada se avaluó de forma manual los espacios interarticulares. Resultados. El espacio condilar anterior en pacientes con y sin disfunción témporo-mandibular no presentó diferencia significativa, p=0,30. La posición condilar tampoco mostró diferencia significativa p=0,58. En pacientes con y sin disfunción témporo-mandibular (sintomáticos) la posición central y posterior del cóndilo (35,2%), pacientes con y sin disfunción témporo-mandibular (asintomáticos) la posición anterior y central fue más significativa (37,0%); seguido de la posición posterior del cóndilo (26,1%). Conclusión. No existe diferencia significativa en la posición condilar y el espacio interarticular en pacientes sintomáticos y asintomáticos.


Abstract Aim. To compare the condylar position and joint space of the temporomandibular join in individuals with and without temporomandibular dysfunction, using cone beam computed tomography (CBCT). Methods. Research Diagnostic Criteria for Temporomandibular Disorders Axis II (n= 50) patients both sexes, age 18 - 27 years. Random sample (n=50) patients equivalent to 100 joints, obtaining: n-25 patients without temporomandibular dysfunction, considered asymptomatic and 25 patients with and without temporomandibular dysfunction, considered symptomatic. By tomography Cone Beam of open mouth temporomandibular joint - closed mouth was manually evaluated the interarticular spaces. Results. The anterior condylar position space and condylar position not significantly different in induvials with and without temporomandibular dysfunction temporomandibular disorders p=0,30 and p=0,58, respectively. Conclusions. There is no significant difference in the condylar position and intraarticular.

RESUMEN: La articulación temporomandibular es una estructura doble y simétrica, con una compleja gama de movimientos. La morfología condilar puede variar dependiendo al sexo, edad, entre otros factores. Se analizaron 100 tomografías computarizadas de haz cónico (50 hombres y 50 mujeres) divididos en 4 grupos. Se identificó diferencias significativas entre las formas aplanadas vs ovaladas del cóndilo y a su vez se encontró diferencia entre los lados derechos vs izquierdo entre hombres y mujeres. La tomografía computada de haz cónico es un método económico y accesible mediante la exposición a bajas dosis de radiación se obtienen imágenes de alta resolución de los tejidos duros de la ATM. El análisis de la tomografía computarizada de haz cónico tiene una precisión aceptable para el diagnóstico de anomalías óseas de la ATM, componentes articulares, integridad del hueso cortical y anomalías óseas subcorticales.


SUMMARY: The temporomandibular joint is a double and symmetrical structure, whit a complex range of movements. Condylar morphology can vary depending on sex, age, among other factors. 100 cone beam computed tomography scans (50 men and 50 women) divided into 4 groups were analyzed. Significant differences were identified between the flattened vs oval forms on the condyle and, in turn a difference was found between the right vs left sides between men and women. Cone beam computed tomography analysis has acceptable accuracy for the diagnosis of TMJ bone abnormalities, articular components, cortical bone integrity and subcortical bone abnormalities.

ABSTRACT

SUMMARY: Unilateral condylar hyperplasia (UCH) is an alteration of the mandibular condyle growth. The aim of this study was to evaluate condyle volume, surface area, and Morphological Index (MI) differences between the affected condyle and an unaffected one in patients with UCH, evaluated through 3D reconstructions cone beam computed tomography (CBCT) images by two open-source softwares. A retrospective cross-sectional study of 16 patients with a certain UCH, 9 females and 7 males with mean age 25.13 ± 6.8 years was made. The image obtained from the CBCT of each condyle were reconstructed using the open-source software 3D SLICER 4.6 ®. The volumetric and area measurements of the 3D reconstruction of the mandibular condyle were made using the open-source soft- ware NETFABB basic 5.0 ®. The mean condylar volume of the hyperplastic condyles was 2.07 ± 1.51 cm3 and the non-hyperplastic condyles was 1.16 ± 0.82 cm3 (p<0.05). The mean area surface of the hyperplastic condyle was 11.77 ± 3.71 cm2 and the non-hyperplasic condyle mean was 8.05 ± 2.17 cm2 (p < 0.05). The mean area surface difference was 3.72 ± 3.57 cm2 (28.0 %). The MI of the hyperplastic condyle was 1.8 ± 0.3 mm and the non-affected condyle was 1.3 ± 0.6 mm (p < 0.05). The use of open-source software for 3D reconstruction with manual segmentation for evaluation of the volume and the condylar surface is a valid tool available to the clinic in the diagnosis and monitoring of patients with condylar hyperplasia.


RESUMEN: La hiperplasia condilar unilateral (HU) es una alteración del crecimiento del cóndilo mandibular. El objetivo de este estudio fue evaluar en pacientes el volumen del cóndilo, el área de superficie y las diferencias del índice morfológico (IM) entre el cóndilo afectado y el no afectado en pacientes, mediante tomografía computarizada de haz cónico (TCHC), por medio de dos softwares. Se realizó un estudio transversal retrospectivo de 16 pacientes con determinada HU, 9 mujeres y 7 hombres con edad media 25,13 ± 6,8 años. La imagen obtenida del TCHC de cada cóndilo se reconstruyó utilizando el software de código abierto 3D SLICER 4.6 ®. Las medidas volumétricas y de área de la reconstrucción 3D del cóndilo mandibular se realizaron utilizando el software de código abierto NETFABB basic 5.0 ®. El volumen condilar medio de los cóndilos hiperplásicos fue de 2,07 ± 1,51 cm3 y el de los cóndilos no hiperplásicos fue de 1,16 ± 0,82 cm3 (p <0,05). La superficie media del cóndilo hiperplásico fue de 11,77 ± 3,71 cm2 y la media del cóndilo no hiperplásico fue 8,05 ± 2,17 cm2 (p <0,05). La diferencia de superficie de área media fue 3,72 ± 3,57 cm2 (28,0 %). El IM del cóndilo hiperplásico fue de 1,8 ± 0,3 mm y el cóndilo no afectado fue de 1,3 ± 0,6 mm (p <0,05). Para el clínico, el uso de software de código abierto en la reconstrucción 3D con segmentación manual, para la evaluación del volumen y la superficie condilar, es una herramienta efectiva en el diagnóstico y tratamiento de pacientes con hiperplasia condilar.

ABSTRACT

Aim: To determine the association between the level of temporomandibular disorder (TMD) and the condylar position in a university population. Material and Methods: A cross-sectional study was carried out in 41 university students between 18 and 27 years old (21±2.28). The level of TMD was determined using the Helkimo index modified by Maglione, whereas the condylar position was found radiographically by lateral scan. The association was evaluated using the Chi-square statistical test. Results: Statistically significant association was found between the TMD level and the condylar position in the female gender (p=0.003). The central condylar position was the most frequent in females (70.00%), while in males the highest frequency of condylar positions was posterior and anterior, 40.48% and 35.71% respectively. In mild TMD, the most frequent condylar position was central (46.34%), whilst non-centric positions were prevalent in moderate TMD, with 2.44%. There was no statistically significant association between the TMD level and the condylar position of the participants, nor in males (p>0.05). Conclusion: The TMD was associated with the condylar position in females of the university population studied, analyzed in lateral temporomandibular joint scans. Non-centric condylar positions were more frequent in the moderate TMD level and centric positions in mild TMD.


Objetivo: Determinar la asociación entre el nivel de trastorno temporomandibular (TTM) y la posición condilar en una población universitaria. Material y Métodos: Se realizó un estudio transversal en 41 individuos universitarios entre 18 y 27 años (21±2.28). Se determinó el nivel de TTM mediante el índice de Helkimo modificado por Maglione, mientras que la posición condilar fue hallada radiográficamente mediante escanografía lateral. La asociación fue evaluada mediante la prueba estadística Chi-cuadrado. Resultados: Se halló asociación estadísticamente significativa entre el nivel de TTM y la posición condilar en el género femenino (p=0.003). La posición condilar central fue la más frecuente en las mujeres con un 70.00% de aparición, mientras que en los hombres la mayor frecuencia de posiciones condilares fue posterior y anterior con 40.48% y 35.71% respectivamente. En el TTM leve la posición condilar más frecuente fue la central con un 46.34% de aparición, mientras que las posiciones no-céntricas lo fueron en el TTM moderado con un 2.44%. No hubo asociación estadísticamente significativa entre el nivel de TTM y la posición condilar en el total de participantes, ni tampoco en el género masculino (p>0.05). Conclusión: Se encontró asociación entre el nivel de TTM y la posición condilar en escanografías laterales de articulación temporomanibular en el género femenino de la población universitaria estudiada, mientras que no se encontró en el total de muestra, ni en el género masculino. Las posiciones condilares no-céntricas fueron más frecuentes en el nivel de TTM moderado y las céntricas en el TTM leve.

OBJECTIVE@#To analyze the biomechanical mechanism of mandibular symphyseal fracture combined with bilateral intracapsular condylar fractures using finite element analysis (FEA).@*METHODS@#Maxillofacial CT scans and temporomandibular joint (TMJ) MRI were performed on a young male with normal mandible, no wisdom teeth and no history of TMJ diseases. The three-dimensional finite element model of mandible was established by Mimics and ANSYS based on the CT and MRI data. The stress distributions of mandible with different angles of traumatic loads applied on the symphyseal region were analyzed. Besides, two models with or without disc, two working conditions in occlusal or non-occlusal status were established, respectively, and the differences of stress distribution between them were compared.@*RESULTS@#A three-dimensional finite element model of mandible including TMJ was established successfully with the geometry and mechanical properties to reproduce a normal mandibular structure. Following a blow to the mandibular symphysis with different angles, stress concentration areas were mainly located at condyle, anterior border of ramus and symphyseal region under all conditions. The maximum equivalent stress always appeared on condylar articular surface. As the angle between the external force and the horizontal plane gradually increased from 0° to 60°, the stress on the mandible gradually concentrated to symphysis and bilateral condyle. However, when the angle between the external force and the horizontal plane exceeded 60°, the stress tended to disperse to other parts of the mandible. Compared with the condition without simulating the disc, the stress distribution of articular surface and condylar neck decreased significantly when the disc was present. Compared with non-occlusal status, the stress on the mandible in occlusal status mainly distributed on the occlusal surface, and no stress concentration was found in other parts of the mandible.@*CONCLUSION@#When the direction of external force is 60° from the horizontal plane, the stress distribution mainly concentrates on symphyseal region and bilateral condylar surface, which explains the occurrence of symphyseal fracture and intracapsular condylar fracture. The stress distribution of condyle (including articular surface and condylar neck) decreases significantly in the presence of arti-cular disc and in stable occlusal status when mandibular symphysis is under traumatic force.

SUMMARY: The study aims to evaluate the morphological variation of mandibular condyle This analysis was composed of 450 digital orthopantomograms taken for routine investigation to assess the morphology of 900 condylar heads. Condylar morphology was classified into four types (oval, bird beak, diamond and crooked finger). Most common occurring shape were evaluated, symmetry of condylar shape and most common combinations of the condylar shapes present in population were assessed. Out of 900 mandibular condyle, type 1(oval) of condyle was the most commonly appearing shape (56 %), followed by type 3 (diamond), type 2(bird beak) and finally type 4 (crooked finger). Symmetry was seen in 74.2 % of the total sample with the oval-oval was the most commonly occurring. Routine use of orthopantomography can assist dentist in assessment of condylar morphology alterations. Oval shape was the most common in all age groups in males and females. More studies on Iraqi population evaluating mandibular condyle are needed to provide standardized values for mandibular anatomical parts.


RESUMEN: El objetivo del presente estudio fue evaluar la variación morfológica del cóndilo mandibular. El análisis se realizó en 450 ortopantomogramas digitales en una investigación de rutina para evaluar la morfología de 900 cabezas condilares. La morfología condilar se clasificó en cuatro tipos (ovalada, pico de pájaro, diamante y dedo torcido). Se evaluó la forma más común de ocurrencia, la simetría de la forma condilar y las combinaciones más comunes en la población. De 900 cóndilos mandibulares, el tipo 1 (ovalado) de cóndilo apareció con mayor frecuencia (56 %), seguido del tipo 3 (diamante), el tipo 2 (pico de pájaro) y finalmente el tipo 4 (dedo torcido). Se observó simetría en el 74,2 % de la muestra total, siendo el óvalo-óvalo el que se presentó con mayor frecuencia. El uso rutinario de la ortopantomografía puede ayudar al dentista a evaluar las alteraciones de la morfología condilar. La forma ovalada fue la más común en todos los grupos etarios en hombres y mujeres. Se necesitan más estudios sobre la población iraquí que evalúen el cóndilo mandibular para proporcionar valores estandarizados para evaluar la anatomía mandibular.

The study of the temporomandibular joint (TMJ) through imaging, is useful for the analysis of intra-articular procedures in view of its complex anatomy. Precise knowledge of the depth at which the TMJ is located is required to achieve an ideal puncture technique. The aim of this study was to measure the depth of the TMJ through magnetic resonance imaging (MRI) in patients with temporomandibular disorders (TMD). A cross-sectional study was conducted, selecting 150 MRI of patients who attended the Polyclinic for TMD and Orofacial Pain. The variables analyzed were: 1) Depth of the right and left TMJ; 2) Age of the patients; and 3) Sex of the patients. Of the total MR, 114 corresponded to women with a median age of 23 years. The median depth of the right TMJ was 17.16 mm and median on the left side was 16.98 mm, there was no statistically significant difference (p> 0.05) but there was a strong correlation (r = 0.842). There were no differences between the depths and the sex of the patients in both the right and left TMJ. There was no correlation between age and depth of TMJ. In conclusion the depth of the right and left condyle are highly correlated, being approximately 17 mm according to the population studied. There is no association between patient age and condylar depth, and there are no differences in average according to sex.


El estudio por imágenes de la articulación temporomandibular (ATM) es útil para el análisis de procedimientos intra-articulares debido a la compleja anatomía que presenta. Se requiere un conocimiento preciso de la profundidad a la cual se encuentra la ATM para una adecuada técnica de punción. El objetivo de este estudio fue medir la profundidad de laATM en relación a la piel a través de resonancia magnética (RM) en pacientes con trastornos temporomandiblaes (TTM). Se realizó un estudio transversal, seleccionando 150 RM de pacientes que asistieron al Policlínico de TTM y Dolor Orofacial. La variables analizadas fueron: 1) Profundidad de la ATM derecha e izquierda; 2) Edad de los pacientes; y 3) Sexo de los pacientes. Del total de RM, 114 correspondían a mujeres con una mediana de edad de 23 años. La mediana de la profundidad de la ATM derecha fue de 17,16 mm y la mediana del lado izquierdo fue de 16,98 mm, no hubo una diferencia estadísticamente significativa (p>0,05) pero si una fuerte correlación (r=0,842). No hubo diferencias entre las profundidades y el sexo de los pacientes tanto en la ATM derecha como en la izquierda. No hubo correlación entre la edad y la profundidad de la ATM. La profundidad de los cóndilos derecho e izquierdo están altamente correlacionados, siendo 17 mm aproximadamente en la población estudiada. No existe asociación entre la edad de los pacientes y la profundidad condilar, y no hay diferencias en promedios por sexo.

ABSTRACT

To evaluate the anterior-posterior (A-P)/medial-lateral (M-L) dimension, and morphology of the mandibular condyle in patients aged 18 to 65 years with Class I skeletal pattern on Cone Beam CT scans. Materials and Methods: Seventy one scans were evaluated using RealScan 2.0 software. The dimension was determined by points A (most anterior in the sagittal plane), P (most posterior in the sagittal plane), M (most interior in the coronal plane), L (most exterior in the coronal plane). The morphology of the condyle was evaluated in two coronal and sagittal planes, being classified as: round, flat, convex or mixed. The size of the condyle was analyzed by descriptive statistics and the morphology by frequency distribution. For the bivariate analysis, the Student's t-test was applied. Results: Measurements were obtained for the A-P diameter of the right condyle (RC) (8.72mm ± 1.25mm) and the left condylar (LC) (8.50mm ± 1.50mm), the M-L diameter of the RC (19.24mm ± 2.03mm) and the LC (18.97mm ± 1.87mm). There were significant differences in the male M-L dimension of the LC compared to the female (p=0.002). The most prevalent morphology of RC (35.21) and IQ (23.94) in the coronal plane was round. Conclusion: The A-P dimension of the right and left condyle is similar in both genders; however, there are differences in the M-L dimension of the left male condyle. The most prevalent morphology of the right and left condyle was round in the sagittal plane with the exception of the coronal plane.


Evaluar la dimensión antero- posterior (A-P)/medio-lateral (M­L), y la morfología del cóndilo mandibular en pacientes de 18 a 65 años con patrón esquelético Clase I en tomografías computarizadas Cone Beam. Material y Métodos: 71 tomografías fueron evaluadas mediante el software RealScan 2.0. La dimensión fue determinada por los puntos A (más anterior en el plano sagital), P (más posterior en el plano sagital), M (más interno en el plano coronal), L (más externo en plano coronal). Se evaluó la morfología del cóndilo en dos planos coronal y sagital, clasificándose en: redonda, aplanada, convexa y mixta. La dimensión del cóndilo fue analizada por estadística descriptiva y la morfología mediante distribución de frecuencias. Para el análisis bivariado, se aplicó la prueba de t de Student. Resultado: Se obtuvieron las medidas del diámetro A-P del cóndilo derecho (CD) (8,72mm ± 1,25mm) y el izquierdo (CI) (8,50mm ± 1,50mm), el diámetro M-L del CD (19,24mm ± 2,03mm) y el CI (18,97mm ± 1,87mm). Hubo diferencias significativas en la dimensión M-L del CI del sexo masculino en comparación al femenino (p=0.002). La morfología más prevalente del CD (35,21) y CI (23,94) en plano coronal fue de tipo redonda. Conclusión: La dimensión A-P del cóndilo derecho e izquierdo es similar en ambos sexos; sin embargo, existen diferencias en la dimensión M-L del cóndilo izquierdo del sexo masculino. La morfología del cóndilo derecho e izquierdo más prevalente fue la redonda en plano sagital a excepción del plano coronal.

OBJECTIVE@#To compare temporomandibular joint (TMJ) morphology and position among skeletal class Ⅱ female adolescents with different vertical patterns using cone-beam CT (CBCT).@*METHODS@#Diagnostic CBCT images of 80 female patients aged 11 to 14 years were assessed retrospectively. According to subspinale-nasion-supramental angle (ANB) and Frankfort horizontal plane-gonion-gnathion angle (FH-GoGn), the participants were categorized into four groups (20 subjects each), i.e. class Ⅰ normal angle (group 1, 0°≤ANB < 4°, 22°≤FH-GoGn≤32°), class Ⅱ low (group 2, ANB≥4°, FH-GoGn < 22°), normal (group 3, ANB≥4°, 22°≤FH-GoGn≤32°) and high angle (group 4, ANB≥4°, FH-GoGn > 32°). Cephalometrics, morphology and position of TMJ were measured in Dolphin software. Using paired-samples t test to analyze TMJ symmetry, One-way analysis of variance (One-way ANOVA) and Chi-square tests to detect differences among the groups. The correlations between cephalometrics and TMJ measurements were also analysed within the skeletal class Ⅱ patients.@*RESULTS@#(1) Analysing TMJ morphologic symmetry, some measurements differed statistically although the mean diffe-rences were negligibly relative to their values. No statistically significant difference was found among the groups though group 4 showed the highest probability of condylar position asymmetry (65%). (2) Comparing group 1 with group 3, statistical difference was found in condylar position (χ2=6.936, P < 0.05) instead of morphologic measurements. Anterior and concentric condylar position were more frequently observed in group 1, yet posterior position was more prevalent in group 3. (3) In groups 2, 3, and 4, statistically, group 2 had the deepest glenoid fossa depth (H2&4=10.517, P=0.002), biggest superior (LSD-t2&3=3.408, LSD-t2&4=5.369, P < 0.001) and lateral (LSD-t2&3=2.767, LSD-t2&4=3.350, P=0.001) joint spaces, whereas group 4 showed the shortest condylar long axis diameter (H2&4=13.374, P < 0.001), largest glenoid fossa vertical distance (LSD-t2&4=4.561, P < 0.001, LSD-t3&4=2.713, P=0.007), smallest medial (LSD-t2&4=-4.083, P < 0.001) and middle (LSD-t2&4=-4.201, P < 0.001) joint spaces. The posterior condylar position proportion gradually increased from groups 2 to 3 to 4. Correlation analysis revealed ANB correlated with anterior joint space positively (r=0.270, P=0.037) and condylar long axis angle negatively (r=-0.296, P=0.022). FH-GoGn correlated with superior (r=-0.488, P < 0.001), posterior (r= -0.272, P=0.035), mesial (r=-0.390, P=0.002), middle (r=-0.425, P=0.001), and lateral (r=-0.331, P=0.010) joint spaces, articular eminence inclination (r=-0.259, P=0.046), as well as condylar long axis diameter (r=-0.327, P=0.011) negatively, and glenoid fossa depth (r=0.370, P=0.004) positively.@*CONCLUSION@#TMJ characteristics of skeletal class Ⅱ sagittal pattern mainly reflected in condylar position rather than morphology. TMJs of different vertical patterns differed more in joint spaces, position of condyle and glenoid fossa than in morphologic measurements. Vertical position of glenoid fossa and proportion of posterior condyle increased gradually from hypodivergent to hyperdivergent. Highest glenoid fossa position, maximum ratio of posterior positioned condyle, smallest joint spaces, shallowest glenoid fossa depth, and narrowest condylar long axis diameter were found in skeletal class Ⅱ high angle group, which means that patients with this facial type have considerable joint instable factors, and we should especially pay attention when orthodontic treatment is carried out on them.

ABSTRACT

Temporomandibular disorder (TMD) is a multi-etiologic disability, accompanied by a wide range of symptoms, requiring a variety of therapies, with TMJ reconstruction considered the most severe. TMJ surgery is associated with many morbidities and limitations, especially in the elderly. Previous studies have determined the shape and dimensions of the Condylus mandibulae in children and adults, but a range of measurements in the elderly population has yet to be created. This study aims to establish a range of measurements and the morphology of the Condylus mandibulae in an older population using micro CT. 14 cadaveric Condylus mandibulaes (8 male, 6 female; average age, 83±8.6) were scanned with microCT and measured using Amira 4.1.1 modeling software. The anteroposterior length (LAP), mediolateral width (WML) and height (H) of each Condylus mandibulae was measured from 5 equidistant slices in the coronal, sagittal and axial planes. Additionally, each Condylus mandibulae was measured at its greatest anteroposterior length, mediolateral width and height using digital callipers on the cadaveric specimen. This study is the first to accurately describe the morphology of the Condylus mandibulae in an elderly population. The length and width measurements of digital calliper (10.51 ± 0.87; 18.33 ± 2.35) and corresponding micro-CT (11.46 ± 1.60; 17.62 ± 2.05) both showed high consistency and reliability. Our findings lay the foundation for the creation of an off-the-shelf Condylus mandibulae prosthesis or anatomically shaped Condylus mandibulae scaffold for elderly patients.


El trastorno temporomandibular (TTM) es una discapacidad multi-etiológica, acompañada por una amplia gama de síntomas, que requieren una variedad de terapias, entre las cuales la reconstrucción de la articulación temporomandibular (ATM) es considerada una de las terapias más graves. La cirugía de la ATM se asocia con muchas morbilidades y limitaciones, especialmente en adultos mayores. Estudios previos han determinado la forma y las dimensiones del cóndilo de la mandíbula en niños y adultos, no obstante aún no se han identificado estas mediciones en una población de adultos mayores. Este estudio tiene como objetivo establecer un rango de mediciones y la morfología del cóndilo de la mandíbula en una población de adultos mayores mediante microtomografía computarizada (microCT). Se escanearon, con microCT, 14 mandíbulas cadavéricas (8 hombres, 6 mujeres; edad promedio: 83 ± 8,6 años), y se realizaron mediciones utilizando el software de modelado Amira 4.1.1. De cada cóndilo de la mandíbula se midió: longitud anteroposterior (LAP), ancho mediolateral (AML) y altura (A), a partir de 5 cortes equidistantes en los planos coronal, sagital y axial. Además, también se utilizaron calibradores digitales para realizar estas mismas mediciones. Este estudio es el primero en describir con precisión la morfología del cóndilo de la mandíbula en una población de adultos mayores. Las medidas de longitud y anchura del calibrador digital (10,51 ± 0,87; 18,33 ± 2,35) y el micro-CT correspondiente (11,46 ± 1,60; 17,62 ± 2,05) mostraron una alta consistencia y confiabilidad. Nuestros hallazgos sientan las bases para la creación de una prótesis de cóndilo de la mandíbula o un andamio condilar de forma anatómica para pacientes de edad avanzada.

ABSTRACT

This study aimed to evaluate the effects of bisphosphonates on the mandibular bone. Bisphosphonates are drugs which are commonly used in the treatment of many diseases related to bone metabolism such as osteoporosis, breast cancer capable of bone metastasis, prostate and lung cancer and bone cancer such as multiple myeloma. Our study group consisted of a total of 100 panoramic radiographs which were obtained from the examinations of 50 individuals using bisphosphonate and 50 individuals in the control group who applied for routine dental examination to the Department of Oral and Maxillofacial Radiology of Akdeniz University Dentistry Faculty between years 2015 and 2016.The calculations of the mandibular cortical thickness (MCT), mandibular cortical index (MCI), panoramic mandibular index (PMI), condylar angle (CA), gonial angle (GA), antegonial angle (AGA), antegonial depth (AGD) and antegonial index (AGI) were made for each patient. It was found that both left and the right MCT and only the left PMI were affected by age. Only the left AGA and both the left and right MCT and AGD were affected by gender. The left and right AGI measurements of the patients using bisphosphonates were statistically lower than those of the individuals in the control group. Our results suggested that bisphosphonates had various effects on the jaw bones. However, further comprehensive studies need to be made to evaluate the longterm effect of bisphosphonates on bone metabolism.


Este estudio tuvo como objetivo evaluar los efectos de los bifosfonatos en el hueso mandibular. Los bifosfonatos son medicamentos que se usan comúnmente en el tratamiento de muchas enfermedades relacionadas con el metabolismo óseo, como la osteoporosis, el cáncer de mama, metástasis óseas, cáncer de próstata y pulmón y el cáncer de hueso como el mieloma múltiple. Nuestro grupo de estudio consistió en un total de 100 radiografías panorámicas que se obtuvieron de los exámenes de 50 individuos que utilizaron bisfosfonato y 50 individuos en el grupo de control que solicitaron un examen dental de rutina al Departamento de Radiología Oral y Maxilofacial de la Facultad de Odontología de la Universidad de Akdeniz, entre los años 2015 y 2016. En cada paciente se realizaron los cálculos del grosor cortical mandibular (GCM), índice cortical mandibular (ICM), índice mandibular panorámico (IMP), ángulo condilar (AC), ángulo gonial (AG), ángulo antegonial (AAG), profundidad antegonial ( PAG) y el índice antegonial (IAG). Se encontró que tanto el GCM izquierdo como el derecho y solo el IMP izquierdo estaban afectados por la edad. Solo el AAG izquierdo y el GCM izquierdo y derecho y el AGD fueron afectados de acuerdo al sexo. Las mediciones de IAG izquierdo y derecho de los pacientes que utilizan bifosfonatos fueron estadísticamente más bajas que las de los individuos en el grupo de control. Nuestros resultados sugirieron que los bifosfonatos tienen varios efectos en los huesos de la mandíbula. Sin embargo, es necesario realizar estudios más exhaustivos para evaluar el efecto a largo plazo de los bifosfonatos en el metabolismo óseo.

ABSTRACT

RESUMEN: La hiperplasia condilar (HC) es un término genérico de una condición patológica que se utiliza para describir situaciones que causan el crecimiento excesivo y sobredesarrollo del cóndilo mandibular, repercutiendo así también en la mandíbula, es la responsable de alrededor del 50 % de las asimetrías faciales y se presenta con mayor frecuencia entre los 11 y 30 años de edad. Se presenta un caso clínico de una paciente femenina de 21 años de edad que presentaba hiperplasia condilar unilateral izquierda con compensación alveolodentaria. Se realizó condilectomía alta con abordaje endoaural para retirar 5 mm de la parte superior del cóndilo y osteotomía mandibular mediante abordaje intraoral circunvestibular, con disección y preservación del nervio dentario inferior, retirando 8 mm del aspecto inferior de la mandíbula de acuerdo a los requerimientos estéticos. El brindar un tratamiento adecuado a la hiperplasia condilar enfocado a corregir las secuelas tanto funcionales como estéticas es de gran beneficio al paciente ya que le permite mejorar su calidad de vida, el correcto diagnóstico es vital para poder planear un tratamiento exitoso.


ABSTRACT: Condylar hyperplasia (CH) is a generic term for a pathological situation that is used to describe conditions that cause excessive growth and overdevelopment of the mandibular condyle and also impacting on the jaw, this bone formation is responsible about of 50 % of all deformities facial and it occurs most frequently between 11-30 years old. This case report is about a 21-years old female who showed unilateral condylar hyperplasia of left side with alveolodentary compensatory. Was carried out high condilectomy through an endoaural approach removing 5 mm of the upper condyle and the mandibular osteotomy was performed through intraoral approach with dissection and preservation of the inferior alveolar nerve, removing 8 mm of the inferior aspect of the mandible according to the aesthetic requirements. An appropriate treatment for the condylar hyperplasia focused on correcting the side effects such as functional or aesthetic it's of great benefit for the patient because it allows improve their quality of life. A correct diagnosis is very important to planning a successful treatment.

Las imágenes radiográficas 2D, como la radiografía panorámica, han sido utilizadas a través de los años como exámenes iniciales para la evaluación de la asimetría mandibular. El primero en realizar medidas sobre cóndilos y ramas mandibulares fue Habets, en 1988, y concluyó que la existencia de diferencias entre las alturas condilares podrían ser el inicio de trastornos temporomandibulares. En los últimos estudios revisados para el propósito de este trabajo, se han creado nuevas técnicas en 2D, pero además se ha desarrollado la tecnología de imágenes 3D, cuyo aporte a los datos cuantificables es fundamental para el tema estudiado. Revisando artículos publicados sobre las asimetrías mandibulares, identificando la frecuencia, las características imagenológicas y estableciendo las ventajas y limitaciones que se presentan en pacientes desdentados parciales mediante el diagnóstico de la asimetría mandibular en técnicas realizadas en imágenes 2D y 3D. 2D radiographic imaging, such as panoramic radiography, has been used over the years as an initial examination for the evaluation of mandibular asym-metry (MA), and the first to perform measurements of condyles and mandib-ular branches was Habets in 1988, who concluded that differences between condylar heights could mark the begin-ning of the development of temporoman-dibular disorders. In recent years, new 2D techniques have been created, but in addition new 3D image technology has been developed, and it is this contribution to quantifiable data which is the focus of this study: the review of published arti-cles on the frequency and imaging char-acteristics of mandibular asymmetries in partially edentulous patients by means of diagnosis using 2D and 3D images; iden-tification through bibliographic research, describing the most commonly used methods for MA diagnosis using 3D images, and establishing the advantages and limitations of MA diagnosis through 2D and 3D techniques. ABSTRACT

La simetría mandibular puede alterarse por diversos motivos. Variados estudios buscan definir parámetros de "normalidad" y para ello se han descrito técnicas que utilizan la radiografía panorámica (RPN). El objetivo de este estudio fue determinar índices de simetría mandibular/condilar en una muestra de pacientes chilenos. Se realizó un estudio descriptivo de corte trasversal en RPN digitales obtenidas de la Clínica Odontológica de la Universidad de La Frontera. Se aplicaron criterios de inclusión: pacientes mayores de 18 años, dentición permanente completa hasta el primer molar; y de exclusión: signos radiográficos de alteraciones óseas y/ o cirugía mandibular, cóndilos no visibles, ortodoncia. Se utilizó el software ImageJ para dibujar rectas y medir la altura de la rama y cóndilo mandibular del lado derecho e izquierdo de acuerdo a la técnica propuesta por Kjellberg et al. (1994). Con estas medidas se calculó el índice de simetría entre altura condilar y rama (S1); y entre altura condilar y mandibular (S2). 235 RPN digitales fueron analizadas (95 hombres; 140 mujeres) con edad promedio de 24,98±7,32 años. El promedio de S1 fue de 93,74 % (±3,85) en hombres y 93,41 % (±5,77) en mujeres, mientras que para S2 fue de 90,50 % (±6,51) en hombres y 90,27 % (±7,80) en mujeres, sin diferencias significativas para ninguno de ambos índices (p=0,347 y p=0,233, respectivamente). En cuanto a la edad, tampoco se encontraron diferencias significativas en el promedio de los índices S1 y S2 para cada rango etario (p=0,594 y p=0,669, respectivamente). Mediante este estudio se concluye que hay un alto porcentaje de pacientes con simetría mandibular/condilar en ambos sexos y en diferentes rangos etarios, esto sin considerar el estado de salud o presencia de alteraciones morfológicas y/o funcionales.


Mandibular symmetry may be modified by various conditions. A number of studies have attempted to define parameters of "normality" and several techniques have been described using digital orthopantomography (OPG). The aim of this study was to determine mandibular/condylar symmetry indexes in a sample of Chilean patients. A cross sectional descriptive study in digital OPG obtained from the Dental Clinic of Universidad de La Frontera was performed. Inclusion criteria applied were: Patients older than 18 years, complete permanent dentition up to the first molar. Exclusion criteria were also applied: Radiographic signs of bone disease and/or mandibular surgery, not visible condyles, orthodontics. ImageJ software was used to draw lines and measure the height of the ramus and mandibular condyle on the right and left side with the technique postulated by Kjellberg (1994). With these measurements, the symmetry index between condylar height and ramus (S1) and between condylar and mandibular height (S2) were calculated. The study analyzed 235 digital OPG (95 men, 140 women) with an average age of 24.98 ± 7.32 years. The S1 average was 93.74 % (± 3.85) for men and 93.41 % (± 5.77) for women, and for S2 it was 90.50 % (± 6.51) for men and 90.27 % (± 7.80) for women, without significant differences for either index (p=0.347 and p=0.233, respectively). No significant differences were found in reference to age, for indexes S1 and S2 (p=0.594 and p=0.669, respectively). Therefore, it is concluded that there is presence of condylar/mandibular symmetry in both sexes and in age ranges, when measuring digital OPGs without considering health status or presence of any morphological or functional alteration.

ABSTRACT

ABSTRACT: Bifid or trifid mandibular condyle is an extremely rare entity, of unknown etiology, although it has been associated with trauma in the condyle growth center. The development of imagining techniques has allowed the exact characterization of these alterations, which were incidentally discovered by routine dental x-rays. The aim of this report is to describe a case of a mandibular condyle with a trifid appearance in a twelve year-old individual with a history of mandibular trauma during childhood and a review of the relevant literature.


RESUMEN: El cóndilo bífido o trífido es una entidad extremamente rara de etología desconocida, sin embargo, ha sido asociada con trauma en el centro de crecimiento condilar. El desarrollo de técnicas imagenológicas ha permitido la exacta caracterización de estas alteraciones, las cuales son incidentalmente descubiertas durante radiografías de rutina. El objetivo de este reporte es describir un caso de un cóndilo mandibular con apariencia trífida en un paciente de 12 años de edad con historia de trauma en la mandíbula durante la primera infancia y una revisión de la literatura relevante.

ABSTRACT

Abstract Purpose: To evaluate the bone structure of the mandibular condyle through an animal model, after ovariectomy. Methods: Thirty-six female Wistar rats were divided into 2 groups. The OVX group was submitted to bilateral ovariectomy, the SHAM group underwent the same surgical treatment, but without removal of the ovaries. After 90, 105 and 135 days after surgery, six animals from each group were submitted to euthanasia and the part containing the condyle was removed. Results: The microscopic analysis shows an increase in marrow spaces over time in the OVX group. The morphometric study shows reduction in the amount of bone tissue in the OVX group 135 days period in comparison with for the initial period (90 days) (p <0.05, ANOVA, Tukey). Conclusion: The estrogen deficiency also affects the bone structure of the condyle.

ABSTRACT

OBJECTIVE: to compare condylar dimensions of young adults with class II and class III skeletal patterns using cone-beam computed tomography (CBCT). MATERIALS AND METHODS: 124 CBCTs from 18-30 year-old patients, divided into 2 groups according to skeletal patterns (Class II and Class III) were evaluated. skeletal patterns were classified by measuring the ANB angle of each patient. the anteroposterior diameter (A and P) of the right and left mandibular condyle was assessed from a sagittal view by a line drawn from point A (anterior) to P (posterior). the coronal plane allowed the evaluation of the medio-lateral diameter by drawing a line from point M (medium) to L (lateral); all distances were measured in mm. RESULTS: In class II the A-P diameter was 9.06±1.33 and 8.86±1.56 for the right and left condyles respectively, in class III these values were 8.71±1.2 and 8.84±1.42. in class II the M-L diameter was 17.94±2.68 and 17.67±2.44 for the right and left condyles respectively, in class III these values were 19.16±2.75 and 19.16±2.54. CONCLUSION: class III M-L dimensions showed higher values than class II, whereas these differences were minimal in A-P.
